About CarePath
Our mission
CarePath operates an online platform that connects individuals with assisted living communities, independent living facilities, memory care providers, and other senior care resources across Greater Pittsburgh. Our services include facility research, personalized recommendations, consultation, referral coordination, and technology-based care navigation.
Choosing senior living is often urgent, emotional and confusing. Our mission is to make that decision clearer: organized information, honest labels about what is and isn't confirmed, and practical help when families want it.
Who we help
We work with older adults planning ahead, adult children supporting a parent, spouses and partners, and friends or professionals helping someone find the right setting. Some families are exploring options months in advance; others need help quickly after a hospital stay or a change at home.
Anyone can browse the directory and compare communities for free, without creating an account.
How navigation works
Step 1
Needs assessment
We start with a conversation about daily routines, what help is needed today, and what may change. We do not ask for diagnoses or medical records.
Step 2
Location, budget and care requirements
We narrow options by neighborhood, travel distance for family, monthly budget, and the level of care involved — independent living, personal care or assisted living, memory care, or skilled nursing.
Step 3
Impartial options
We present communities that fit those requirements, drawn from our directory of sourced listings, and explain the trade-offs plainly.
Step 4
Side-by-side comparison
Families can compare two to four communities on care levels, services, accessibility, visiting and published costs.
Step 5
Calls and tours
On request, we help you prepare questions and coordinate calls or tours. We share your details with a community only with your separate permission.
Step 6
Follow-up
After visits, we help you review what you learned and decide on next steps. The decision always stays with you and your family.
Levels of senior living
- Independent living
- Private apartments or homes for active older adults, often with dining, housekeeping and social programs, but little or no personal care.
- Personal care / assisted living
- Help with everyday activities such as bathing, dressing and medications, in a residential setting. In Pennsylvania these are licensed as personal care homes or assisted living residences.
- Memory care
- Secure, structured settings designed for people living with Alzheimer's disease or other forms of dementia.
- Skilled nursing
- Licensed nursing facilities offering 24-hour nursing care and rehabilitation, regulated by the state and, when certified, by Medicare.
Many campuses offer more than one level. Each community profile lists the care levels its own website or public records describe.
What makes us different
- Sourced, not guessed. Addresses, phone numbers, care types and prices come from community websites and public records, with links and the date each was checked.
- Honest gaps. When something isn't published — current occupancy, staffing ratios, pricing — we say so instead of filling it in.
- Clear labels. Our "Most Recommended" picks and featured stars are CarePath's editorial choices and are always labeled that way, separate from government ratings or online reviews.
- Your consent first. We never pass your information to a community without your separate permission.
Pricing
Browsing the directory, viewing profiles and using the comparison tool are free.
For families who want one-on-one help, we offer an optional $99 personalized consultation: a review of your needs, a tailored shortlist, and help preparing for calls and tours. The fee is for the consultation itself — it is not a deposit, and it does not reserve a place at any community.

CarePath is an independent referral and navigation service. We are not a licensed medical or healthcare provider, and we do not provide medical advice, care or assessments.
We do not guarantee placement, availability, admission or pricing at any community, and listing a community is not an endorsement or guarantee of its care. Always confirm details directly with the provider. Read our full disclosures.
Data practices
Every community profile shows where each fact came from — the community's own website, a state record, or a federal source such as Medicare — and when it was last checked. Where an official rating exists, we show it with its source rather than replacing it with our own.
Information you share through our intake or consultation forms is used to help you, stored securely, and not shared with communities without your consent. See our privacy policy and methodology.
